COMMITTEE HANDBOOK

The Committee Handbook is your guide to understanding the requirements and guidelines for running a committee, handling and reconciling committee funds and managing funds and budgets. All committee chairpersons are required to read and understand the information in this handbook prior to beginning work on a committee.

I'm a chairperson, now what?!

1. Confirm the date of your event with Diana Puzzo by e-mail (dpuzzo@smithtown.k12.ny.us) or phone.

2. Complete FORM B (can be found in the main office or sent home with your child if you are unable to get to the school) for use of School Facilities at least 30 days before the event.

3.  Confirm your budget with Tom Doherty (tdoherty@tlctransport.com).

4. Submit your event flyer to Corey Doherty (corw721@yahoo.com) for approval at least 30 days in advance.

5.  Send final flyer or email to Carisa Wellenreuther (carisarhatigan@gmail.com) and Mr. Furey (mfurey@smithtown.k12.ny.us) to be posted or sent out to the community via email.

6.  If you collect money, it must be submitted within 3 days to Tom Doherty.  Cash receipt form is on the website.

7.  If you require reimbursements, save all receipts and use reimbursement form.

After an event this form should be printed and completed and inserted in the folder for future chairperson reference.

Prior to major events (girls choice, boys choice, holiday breakfast, chinese auction etc) this form must be completed and submitted to the President. No flyers will be approved without the Event Proposal Form.

This form is used to indicate expenses and request reimbursements.

After an event or fundraiser this form MUST accompany all funds handed over to the treasurer.

SPECIAL RULES FOR BUILDING USE AFTER SCHOOL HOURS (Form B)

  • If your event is scheduled in the evening or on the weekend, you will need to complete a Form B. The Form B alerts the school and the district of your desire to use the school building and/or school grounds for your event.
  • The Form B is obtained in the main office from Mrs. Smith and must be signed by school principal, Mr. Matthew Furey AT LEAST 2-3 WEEKS PRIOR to the event. (Note: most committee folders contain a copy of the Form B which must be completed and submitted as soon as a date is approved.)
